Individual Awards

Download the entry kits below for the categories you wish to enter. 

The MFAA individual awards recognise individuals who have made outstanding achievements in the industry.

Entrants must prepare a separate submission for each award they enter, which will be assessed by a panel of judges. Winners will be recognised at each State Excellence Awards with the overall national winner announced at the National Excellence Awards.

Downloading Instructions

1. Click the Download button on the entry kit you wish to download

2. Once the document opens you will need to go to File>Download. If multiple documents download one at a time

3. For the word documents you will need to enable editing in order to start writing your submission. 

4. Then save to your computer
